Welcome to the forum kknowles!

I'm not an accountant so I can only point to info at the IRS site. Have a look at this page:

http://www.irs.gov/publications/p503/ar02.html

Form 1040. You must complete Form 2441 and attach it to your Form 1040. Enter the credit on your Form 1040, line 47.

Form 1040A. You must complete Schedule 2 (Form 1040A) and attach it to your Form 1040A. Enter the credit on Form 1040A, line 29.

Try out Tax Act Online - the Standard Edition is free and can help you if you plan to send in your tax return by mail. They charge a minimal fee ($7.95) for e-filing but you need to meet certain basic criteria in order to be able to e-file - so check to see if you are eligible. The step by step process eliminates all the juggling you need to do to keep up with the laws. There is a process that checks and highlights errors.
