What to do if your website is banned by the search engines for black hat practices?

Get a new domain and start over, this time without the black hat practices.

Seriously.

You can go back and undo every bad thing that was done and submit a request for reinclusion, but there's no guarantee that will work, and you'll have to work even harder to get your site back in Google's good graces than if you were to just start from scratch. And they're going to be going over your site with a fine-toothed comb for a long time to come, because they will know you have a history of engaging in black hat tactics. Much easier to just start fresh.
